Vitamin D Might Matter More for Your Brain Than Your Bones

Could something as simple as a vitamin deficiency be quietly shaping your risk of dementia decades down the line? That is the question a team of researchers set out to answer, and the numbers they found are hard to ignore.

Dementia already affects tens of millions of people worldwide, and the World Health Organization expects that number to keep climbing sharply in the coming decades. Low vitamin D levels show up again and again as one of the modifiable factors behind that trend. Researchers pooled data from 22 observational studies covering 53,122 participants across Europe, Asia and North America, published between 2014 and 2024, to get the clearest picture yet of how the two are connected.

The headline number is stark. People with the lowest vitamin D levels had a 49% higher risk of developing dementia than people with the highest levels (relative risk 1.49, 95% CI 1.32 to 1.67). The team also tested whether the relationship holds up gradually rather than just at the extremes, and it does. For every 10 nmol/L increase in blood vitamin D, dementia risk dropped by about 1.2%, in a straight line with no sign of leveling off or reversing.

On its own, that 1.2% is not going to change much for any one person. The real story is what happens at the population level. Vitamin D deficiency is extremely common, and the authors estimate that moving a population from deficient levels (under 25 nmol/L) up to a sufficient range (50 to 75 nmol/L) could translate into a 3.6% to 6% drop in dementia risk overall, a meaningful shift when applied across millions of people.

The association was not equally strong everywhere. Asian cohorts showed the sharpest link, with more than double the risk in the lowest vitamin D group (RR 2.05), while European and North American studies showed a more moderate relationship. Case-control studies also reported stronger associations than long-term cohort studies, which the authors flag as a sign that some of the effect could be inflated by recall bias rather than biology alone.

There is a plausible biological story behind the numbers. Vitamin D receptors are abundant in the hippocampus, the brain region most central to memory, and vitamin D appears to reduce amyloid buildup, limit tau protein changes, calm inflammation in the central nervous system and support blood vessel health, all processes tied to how dementia develops.

None of this proves that low vitamin D causes dementia, and the researchers are explicit about it. This is an analysis of observational studies, the kind that can spot a pattern but cannot rule out that something else entirely, general frailty, less time outdoors, a poorer diet overall, is driving both the low vitamin D and the higher dementia risk at once. Only randomized controlled trials, where people are actually assigned to take vitamin D or not, can settle whether supplementing actually lowers risk. Those trials are still pending.

Until then, the takeaway is modest but reasonable. Getting enough sunlight, eating vitamin D rich foods and correcting an outright deficiency are already recommended for bone and general health, and this analysis adds cognitive health as one more reason not to let levels run low.
