Here’s What May Happen to Your Brain When Magnesium Runs Too Low or Too High

Is more magnesium always better for your brain? A team from the University of Massachusetts Lowell and Tufts University just pulled together everything published on magnesium and cognitive health in adults, and the answer looks a lot more like a narrow window than an open door.

The safest zone for blood magnesium turned out to be surprisingly tight. Researchers pooled five cohort studies tracking magnesium levels in blood, covering populations from the United States, Denmark and the Netherlands, some with over 100,000 participants. People whose serum magnesium sat around 0.85 millimoles per liter had the lowest risk of dementia or cognitive impairment. Fall below 0.75 mmol/L and the risk climbed 43 percent. Climb above 0.95 mmol/L and it rose 30 percent. Both ends of the scale, too little and too much, tracked with worse outcomes, forming a clear U-shaped curve rather than a straight line where more is simply better.

Dietary magnesium tells a messier story. Six cohorts looked at how much magnesium people actually ate, with daily intakes ranging from 116 to 508 milligrams across the different populations studied. Two long-running Japanese cohorts, the Hisayama Study, found the lowest dementia risk at around 180 milligrams a day, again a middle point rather than an extreme. But a cohort from Shanghai found the opposite pattern, with higher magnesium intake linked to a higher dementia risk, while data from the UK Biobank and the Women’s Health Initiative Memory Study found no clear link either way. When researchers combined the numbers, the mathematical hint of a similar U-shape appeared, with a possible sweet spot near 240 milligrams a day, but it stayed short of standard statistical significance.

Supplements barely have any real trial evidence behind them. Only three small randomized trials have tested magnesium pills directly against a placebo for cognitive outcomes, and the results split down the middle. One trial in breast cancer patients found no benefit at all. Another, nested inside a colorectal cancer prevention study, found a real improvement in cognitive scores among adults over 65, an effect the researchers partly traced to changes in DNA methylation near a gene linked to Alzheimer’s risk. A third, small and short, found a memory benefit in people with liver cirrhosis. Three trials is nowhere near enough to draw a firm conclusion either way.

None of this proves that magnesium levels cause the changes in dementia risk. Every one of the studies behind the U-shaped pattern is observational, meaning people with unusual blood magnesium levels might simply differ from everyone else in ways researchers cannot fully account for, from kidney function to overall diet quality to underlying disease that itself alters magnesium levels. The authors are explicit that their confidence in the blood magnesium findings is only moderate, and that the dietary intake evidence is graded as low, largely because different studies contradict each other. What the review does support is that magnesium status probably matters for brain aging, but treating a supplement bottle as a guaranteed shortcut to sharper memory gets ahead of what the data can currently show.
